Annual Membership Fee:

Adults (adult teeth): $399 per year
Children (baby teeth): $299 per year

Your Membership Includes:

2 hygiene visits per year (prophy or perio maintenance as needed)
Comprehensive & periodic exams
Annual X-rays (bitewings, periapical, and panoramic as needed)
1 emergency exam per year (if needed)

15% off all additional treatment, including:

Fillings
Crowns & Bridges
Extractions
Dentures
Root Canals
Additional cleanings beyond the two included
CBCT scans (as needed for treatment planning)
